Position: DSNP Clinical Pharmacist Practitioner Work Authorization: US Citizen/ Green Card Pay: $67 plus/hr (Pay is based on experience) Location: Jacksonville, FL (Remote) ___________________________________________________________ Job Summary: We are looking for a licensed and experienced DSNP Clinical Pharmacist Practitioner for our health care client in Jacksonville, FL. As a Clinical Pharmacist Practitioner, you will apply specialized medical, pharmaceutical, legal, or actuarial knowledge gained from advanced degrees to perform department activities and provide expert advice. You will utilize your discipline-specific knowledge, including theories, practices, and techniques, to conduct comprehensive medication reviews for eligible members and provide clinical recommendations to resolve drug therapy problems. ___________________________________________________________ Education & Experience: ??? PharmD in Pharmacy ??? Current Pharmacy licensure in any state in the USA, with a valid license to practice in Florida, being mandatory. Licensure in other states is considered a bonus. ??? Medication Therapy Management certified or minimum 3 years of MTM experience. ??? PBM or Insurance Plan work experience preferred. ??? Prior D-SNP experience is required. ___________________________________________________________ Responsibilities: ??? Conduct comprehensive medication reviews for eligible members utilize prescription data to complete a Comprehensive Medication Reviews. ??? Communicate with members and providers to resolve drug therapy problems. ??? Provide clinical recommendations pertaining to, but not limited to, missing and duplicate drug therapies, gaps in care, high-risk medications, medication compliance and adherence, drug interactions, therapeutic substitution, and generic substitution based on medication review and reconciliation. ??? Identify and help resolve barriers to adherence; maximize utilization of generics or lower-cost brand alternatives, and reduce polypharmacy. ??? Utilize formulary and member-specific benefits to guide appropriate medication recommendations. ??? Validate and quality check all documents and reports to ensure accuracy. ??? Conduct member-level medication counseling and disease education. ??? Task case nurses with patient discussion points and recommend alternatives to improve member health outcomes to reduce hospitalizations and improve the quality of care for members. ??? Attend regularly scheduled ICT meetings as part of the team. ??? Be available for medication consultations, physician outreach, and provider engagement as needed. ??? Complete comprehensive medication reviews (CMR) for D-SNP members who qualify for the Medication Therapy Management (MTM) program. ___________________________________________________________ Requirements: ???
