The individual works under the supervision of the Senior Pharmacy Manager of Pharmacy Government Programs and Medicare Clinical Pharmacist. Their main responsibilities include making phone calls to members to support them in closing Stars care gaps and addressing medication adherence barriers. They work closely with the Medicare Pharmacy team to achieve the overall pharmacy Stars goal. Some of the specific tasks they perform are: Making outbound phone calls to members to close gaps in care. Fielding incoming member return calls to address gaps in care. Managing daily reports and call lists to prioritize members needing a prescription refill. Identifying and addressing barriers preventing members from adhering to their medications. Identifying adherence issues that require escalation to a pharmacist and facilitating the escalation process. Documenting barriers, actions taken, follow-ups, and outcomes resulting from member interventions. Supporting Clinical Pharmacists with adhoc outreach as needed. Researching alternative phone numbers for members when unable to reach them due to inaccurate contact information. Effectively managing internal and external communications. Collaborating with the MTM team, other pharmacy and Stars team members to achieve the Stars goals. Building rapport and influencing members and their caregivers to improve adherence with prescribed medications. Contacting pharmacies to facilitate prescription refills as needed. Identifying and properly escalating non-clinical pharmacy-related benefit/plan questions from members. The required qualifications for this role are: Pharmacy Technician Certification. Minimum 5 years of direct pharmacy work experience. Minimum 3 years of call center or direct member outreach experience. Preferred qualifications include: 3 years of Medicare Stars programs member outreach experience. In addition to the above, the individual may be assigned additional duties as necessary. They may participate in required MTM Call Center operational meetings and training sessions, provide support to MTM services with pharmacist oversight, respond to requests from beneficiary service and other issues, research demographic information, support pharmacists in ensuring CMS requirements are met, evaluate and ensure accuracy of documentation for Comprehensive Medication Reviews (CMRs), interact with qualified members and medical providers to schedule and complete CMRs, manage peer review cases, handle pharmacy benefit questions, and review and ensure accuracy of reports prior to submission to CMS. | |
