Weekly Pay! $22.22/Hour Shift Details: 08:00 AM to 02:00 PM DESCRIPTION: JOB SUMMARY Under the supervision of the Hematology Care Coordination Supervisor, integrates care for patients by performing complex care coordination activities with precision. This includes, but is not limited to, the coordination of CBCI provider visits, phlebotomies, diagnostic and therapeutic procedures, and infusion therapies. When need arises, will perform other duties as assigned by supervisor. Job duties include: Coordinates all aspects of complex care planning for patients with physicians, APPs, and their Nursing teams to manage outstanding appointments, treatments and testing required for the care of hematology and post-stem cell transplant patients. This requires working with the clinical team to prioritize patient care planning, as well as adjust patient and provider schedules based on clinical status. Demonstrates an advanced understanding for patient populations (hematologic, autologous transplant, allogenic transplant, immune effector cell therapy, and research) in order to appropriately prioritize scheduling, treatment, and testing needs for these patients. Coordinates/communicates all infusion services and testing that will be done outside of CBCI with the appropriate teams/companies to include, but not limited to, P/SL radiology, P/SL CVC, PSL infusion center, etc. This includes providing OncoEMR orders, authorization numbers, referral numbers, demographics and patient availability for testing and infusion care. Coordinates lumbar punctures, bone marrow biopsies and bone marrow harvests with the nursing coordinator teams, provider teams and hospital teams. Participates in planning of complex outpatient follow up care for patients who are discharging from the hospital, in conjunction with the inpatient APP team and the nursing coordinator teams. This includes, but is not limited to: attending daily inpatient nurse charge rounds, working with inpatient floor nurses to secure office visits for patients at CBCI, and working with inpatient APPs to coordinate Spaulding rehab care. Builds monthly OncoEMR provider templates and/or cross-checks accuracy of templates with Supervisor. Maintains monthly provider meeting build spreadsheets and adds all appropriate monthly meetings to provider templates and/or cross-checks accuracy of meetings with Supervisor. Identifies appropriate length of both physician and infusion appointments to include but not limited to chemotherapy, biotherapy, stem cell infusions, research protocols, and antibiotic therapies. Collaborate with the finance team regarding authorization for all infusions and procedures including providing hospital teams with appropriate authorization data and information. Coordinate with physicians and APP to maintain their most current availability. Coordinates and manages physicians and APP meetings to ensure appropriate utilization of provider availability and patient access to the clinic. Liaisons with CBCI MG when gaps in patient accessibility are identified to resolve this gap in care and ensure patient continuity and safety is maintained. Provide excellent customer service to patients visiting the clinic or calling by phone to schedule appointments, treatments and/or testing. Perform service recovery with patients and caregivers. Manage lost-to-follow up process for patients when the clinic is unable to reach the patient for the clinically appropriate follow up time/window. Follow up on all patient cancellations from automated patient call reminders. Demonstrates the ability to work in multiple systems at once in order to manage patient care and provider accessibility. Observes strict HIPPA compliance for patient?s right to privacy when dealing with our patients and their personal medical information. Aware of patient's right to privacy and questions, when needed, regarding release of information. Demonstrates the ability to work independently and manage complex tasks in a fast paced environment. Demonstrates strong ability to execute on business needs and provide follow up as needed. Strong adaptability required to ensure priorities are managed expeditiously. Communicates work status to the Supervisor on a daily basis and informs the Supervisor of any backlogs, delays, or problems. Experience: Two years of oncology care coordination, scheduling and/or healthcare finance experience in outpatient setting preferred. Special Qualifications: Basic understanding of medical, oncology and stem cell transplant terminology. Knowledge of Word and Excel required. Strong organizational and critical thinking skills. Able to communicate effectively with patients, families, physicians, and other health care team members. Able to function as a part of a multidisciplinary team to ensure quality patient care. Degree of supervision required: Involves general guidance and direction by the Front Office Supervisor. Employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures. Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience). Physical Requirements: Requires corrected hearing and vision to within functional ranges. Requires frequent bending, reaching, squatting, kneeling, moving, lifting of equipment up to 50 pounds and sufficient manual dexterity to operate necessary equipment. Must be able to stand and/or sit for long periods of time. General Comments Health Requirements: Proof of Flu Vaccine October 1st ?
