DESCRIPTION

Justification for a new position
Combine the responsibilities of a recently vacated part-time contract position with new responsibilities justified by the emerging need for enhanced support in colleague resource navigation.
Fulfill the Hope Fund?s mission to provide compassion and assistance through high-touch, high-quality referrals for colleagues and their families in times of need. The current model focuses on financial support, which necessitates the inclusion of wrap-around service referrals.
The Hope Fund grant requests require significant collaboration and partnership with colleagues and HRBP. The volume and support requirements maximize the capacity of our three Grant Specialists.
This role, in partnership with the Grant Specialists and HRBPs, would bridge the capacity gap by assuming responsibility for the long and short-term resource management needs that exist for HCA Healthcare colleagues who come to HR and/or the Hope Fund in acute crisis.
Since 2020, grant volumes and workload responsibilities have increased by 25%. As this growth continues, additional support for colleagues is essential.
The Hope Fund approves over 3,000 grants annually. With an average of 12 grants awarded daily, we anticipate the manager will support 3-5 colleagues per day.

Position Overview
The Manager is integral to the grants team, connecting the Hope Fund with essential company and community resources. This individual is responsible for helping colleagues understand and access social and community resources, including housing, healthcare, education, peer support, and other essential needs within HCA Healthcare. The position manages referrals through continuous community engagement efforts and ensures quality partnerships within local markets. The position is a connector between the Hope Fund, CCE?s as subject matter experts on community resources, and colleague support through HR.

Conduct assessments of colleagues' needs, including social, economic, and personal circumstances.
Provide individualized support and guidance to colleagues accessing social services and community resources, which may include long-term case management and financial coaching tools/programs.
Develop and maintain a comprehensive knowledge of available community resources and services, including eligibility criteria, application processes, and contact information
Maintain accurate and confidential records, documentation, and case notes
Participate in team meetings and professional development activities to enhance skills and knowledge in the field of social work and case management


Qualifications
Bachelor?s degree in social work, psychology, counseling, or a related field
Previous experience in case management, social services, or a related field is highly desirable
Strong understanding of social programs, community resources, and support services
Experience with program development and evaluation a plus
Excellent interpersonal and communication skills, with the ability to establish trust with colleagues from diverse backgrounds
Empathetic and compassionate approach to working with individuals facing challenges and crises
Ability to work independently, prioritize tasks, and manage time effectively in a fast-paced environment
Proficiency in computer skills, including Microsoft Office

Year One Success
The Manager will make connections to HRBP, facility social workers, CCE?s, community resources, and other support services to clearly understand opportunities for alternative resourcing.
The manager will align with HCA Healthcare Benefits & Optum support for connections to HCA Healthcare benefits.
The manager will evaluate the intake process for the Hope Fund and determine additional assessments or questionnaires needed for success.
The manager will curate a resource bank, and when possible assess the quality and reliability of each resource. This information will be routinely updated and from credible sources.
To develop the model, the manager will concentrate on South Atlantic, East Florida, and North Florida, each with limited domestic violence shelters.
Initial focus will support domestic violence requests (Approximately 50/year) and essential needs (750/year) primarily.

Resource Pathway
Initial Assessment
Gather detailed information about the colleague?s situation and needs.
Identify the colleague?s specific needs and challenges. This includes direct conversations with the individual and potential assessments or questionnaires.
Planning
Collaboratively, the Manager and colleague set realistic, achievable, and measurable goals. These goals should be employee-centered and tailored to their unique circumstances.
A personalized plan outlines the steps needed to achieve the individual goals. This plan includes specific actions, timelines, and identifying necessary resources and services.
Coordination of Services
The manager identifies and connects the colleague with appropriate resources and services. This may include social services, housing assistance, educational programs, and other community resources.
The manager facilitates referrals to these services, ensuring the colleague meets the eligibility criteria and understands the process of accessing the resource.
Monitor and Evaluate
The Manager provides follow-up and monitors the colleague's progress toward their goals.
Periodically, the Manager reassesses the client?s situation to determine if the plan needs adjustments.
The manager and colleague review the progress made and formally close the case.
